ABSTRACT

RESUMO Objetivo: mensurar a eficácia da perda de peso, analisar a evolução de comorbidades, investigar a qualidade de vida e avaliar o protocolo BAROS (Bariatric Analysis and Reporting Outcome System) no pós-operatório de pacientes submetidos à cirurgia bariátrica. Métodos: estudo transversal e quantitativo, com força de verdade de 95% (P=0,05), de 103 pacientes submetidos à Gastrectomia Vertical SLEEVE (40) e à Derivação Gástrica em Y de Roux (63), a partir de quatro meses de pós-operatório. A pesquisa foi realizada no Ambulatório de Cirurgia Geral do Hospital das Clínicas da Universidade Federal de Pernambuco, tendo sido utilizado o protocolo BAROS. Resultados: a maioria dos pacientes era do sexo feminino (89,3%). A média de idade foi de 44,23 anos. A média de perda percentual do excesso de peso foi de 69,35%. A média de tempo de seguimento pós-cirúrgico foi de 41,87 meses (±37,35). As comorbidades com maior percentagem de resolução foram: apneia do sono (90,2%), diabetes (80,7%) e hipertensão (70,8%). As complicações mais frequentes foram queda de cabelo (79,6%), deficiência nutricional (37,9%) e anemia (35%). O protocolo BAROS demonstrou que a qualidade de vida foi avaliada de forma positiva em 93,2% dos casos. O questionário Moorehead-Ardelt demonstrou que a qualidade de vida "melhorou" ou "melhorou muito" para 94,1% dos pacientes. Conclusão: a cirurgia bariátrica demonstrou ser um procedimento eficaz no tratamento da obesidade mórbida e no controle das comorbidades. A análise da qualidade de vida foi avaliada de forma positiva através do protocolo BAROS.


ABSTRACT Objective: to assess the efficacy of weight loss, the evolution of comorbidities, the quality of life and the BAROS protocol (Bariatric Analysis and Reporting Outcome System) in the postoperative period of patients undergoing bariatric surgery. Methods: we conducted a cross-sectional, quantitative study with 95% true strength (P=0.05), with 103 patients submitted to SLEEVE Vertical Gastrectomy (40) and Roux-en-Y Gastric Bypass (63), from four months after surgery. We carried out the research at the Ambulatory of General Surgery of the Clinics Hospital of the Federal University of Pernambuco, using the BAROS protocol. Results: the majority of the patients were female (89.3%). The mean age was 44.23 years. The mean percentage loss of excess weight was 69.35%. The mean postoperative follow-up time was 41.87 months (±37.35). The comorbidities with the highest percentage of resolution were sleep apnea (90.2%), diabetes (80.7%) and hypertension (70.8%). The most frequent complications were hair loss (79.6%), nutritional deficiency (37.9%) and anemia (35%). The BAROS protocol demonstrated that patients positively evaluated quality of life in 93.2% of the cases. The Moorehead-Ardelt questionnaire showed that quality of life "improved" or "improved greatly" for 94.1% of patients. Conclusion: bariatric surgery has been shown to be an effective procedure in the treatment of morbid obesity and in the control of comorbidities. Quality of life analysis was evaluated positively through the BAROS protocol.

ABSTRACT

Introduction: Obesity is a public health problem and Chile prevalence has increased in recent years, showing a higher percentage of overweight in adult men and of obesity in adult women. In this context, bariatric surgery appears to be a feasible, alternative. Objective: To describe meanings and experiences of adult patients who have undergone bariatric surgery in private clinics. Method: A qualitative approach based on Grounded Theory was used. The sample selection was made trough key informants and snowball, and was composed of 8 women and 3 men. To collect data semi-structured in depth interviews were used, and data analysis was performed trough open coding. Results: Three categories emerge from descriptive analysis: 1) Motivations and expectations regarding bariatric surgery; 2) Experiences in relation to overweight, and 3) Experiences in relation to food. Conclusions: Further investigation is needed from the subjectivity of the patients themselves, given the impact that obesity and bariatric surgery hason people’s lives.


Introducción: La obesidad es un problema de salud pública y en Chile ha ido en aumento en los últimos años, observándose mayor porcentaje de sobrepeso en adultos hombres y de obesidad en adultas mujeres. En este contexto, la cirugía bariátrica surge como posible alternativa. Objetivo: Describir los significados y vivencias de pacientes adultos que han sido sometidos a cirugía bariátrica en clínicas privadas. Método: Se usó un enfoque cualitativo, basado en la Teoría Fundamentada. La selección de la muestra se realizó a través de informantes clave y bola de nieve, y quedó conformada por 8 mujeres y 3 hombres. Para recoger los datos se utilizaron entrevistas en profundidad semi-estructuradas, y para el análisis se usó la codificación abierta. Resultados: Del análisis descriptivo se obtuvieron tres categorías: 1) Motivaciones y expectativas en relación a la cirugía bariátrica; 2) Vivencias en relación al sobrepeso, y 3) Vivencias en relación a la comida. Conclusiones: Es necesario continuar investigando desde la subjetividad de los propios pacientes dado el impacto que la obesidad y la cirugía bariátrica tienen en la vida de las personas.

ABSTRACT

Background: Gastric cancer is the first cause of death by cáncer in Chile. Quality of Life is a multidimensional construct that explores functionality and well-being, including physiological, psychological and social aspects. Aim: To assess Quality of Life of patients operated for gastric cancer. Patients and methods: The European Organization Research and Treatment of Cáncer Quality of Life Questionnaire (EORTC QLQ-30), translated into Spanish, was applied to 33 patients, aged 42 to 82 years (25 males), subjected to curative total or subtotal gastrectomy for gastric cancer, between January 2004 and December 2006. Results: The average lapse from the moment of the surgical intervention to the interview, was 52.2 weeks. Fifty five percent perceived their Quality of Life in the "good" category. Male patients and those with less than 6 months of surgery, obtained better scores in psychological aspects of quality of life. Conclusions: A high percentage of patients operated for gastric cancer qualified their quality of life as good in spite of the severity of the underlying disease and invasiveness of the intervention. This is probably explained by the importance of psychological factors that influence quality of life.
